I would like to recommend that {Employee Name} be dismissed from employment immediately due to {his/her} insubordinate attitude and behavior.

I am normally pretty flexible when it comes to reassigning material if an employee finds it difficult or accepting late work if it's necessary. However, {Employee Name} persists in refusing to complete {task/duty} on a regular basis. {He/she} neither submits a deferral request nor apologizes for not completing work. When I've spoken to {him/her} about it, {he/she} was openly hostile with me.
